What is Skilled Care? Skilled Care is care that can only be provided by or under the supervision of licensed medical personnel such as a registered nurse (RN) or licensed practical nurse (LPN). It may also include care from a physical therapist, occupational therapist, or nutritionist. It is distinct from other care provided in the home because it is medical care that might otherwise be provided in a clinic or hospital setting and very often prevents hospitalization or re-admittance. CareAparent skilled professionals perform a variety of duties in the home, using their training, experience, and judgment to deliver specialized, client-centered care.

Skilled Nurse Visits

… a licensed medical professional, either registered nurse (RN) or licensed practical nurse (LPN), is visiting regularly to manage and improve clinical outcomes for clients in the home while monitoring their physical and mental health conditions.

Medication Management

… good health is following a medication schedule and understanding the risk associated with each medication to avoid unnecessary admission or rehospitalization. RNs or LPNs can assist in managing medications and schedules, set up medications in boxes and/or dispensers, monitor and order refills, and ensure medications are being taken in the proper dose and at the correct times each day.

INR Checks and Diabetes Management

… you or your loved one’s INR measure. We work directly with your primary care physician to report your INR results and will make any necessary medication changes. Our RNs and LPNs can monitor you or your loved one’s diabetic condition by proposing tips for controlling blood sugars, creating and logging blood sugars, and offering suggestions for healthy food choices.

Monitoring Vital Signs & Oxygen

… blood pressure, temperature, oxygen saturation, and pulse is an important way to monitor general health and specific health conditions such as heart disease or hypertension. Nurses are available to take and record vital signs on a regular basis and report any concerns or changes to your primary care physician.

Catheter and Ostomy Cares

… and LPNs to provide ostomy and catheter care. These clinicians perform regular maintenance and monitor changes to avoid complications. They are also passionate about educating and training you or a family member about how to perform or assist with catheter and ostomy cares.

Wound Care

… management and treatment for our clients with complex, chronic and acute wounds. Providing in-home Wound Care allows healing, improved outcomes and decreased risk of infection in the comfort and control of the home. All care has oversight by on-staff Wound Care Certified Nurse.
