INR Check and Diabetes Management
Monitoring and management made easy. Our RNs and LPNs work directly with primary care physicians to report INR results and make any necessary medication changes. We also can monitor diabetic conditions by proposing tips for controlling blood sugars, creating and logging blood sugars, and offering suggestions for healthy food choices. Checks and management are possible at home.
Monitoring Vital Signs and Oxygen
Making vital sign monitoring convenient. Our RNs and LPNs are available to manage vital signs such as blood pressure, temperature, oxygen saturation and pulse for both general health and also for specific health conditions like heart disease or hypertension. Consistent monitoring and, when necessary, reporting to a primary care physician is possible at home.
Catheter and Ostomy Cares
Specialized help is available for catheter and ostomy care. Our RNs and LPNs are able to perform regular maintenance and monitor changes to avoid complications. And, they are able to educate and train you or a family member to perform or assist with catheter and ostomy care so it can be managed at home.
Wound Care
Take the stress away from wound care. Our RNs and LPNs, with oversight by an on-staff Wound Care Certified Nurse, can help manage and treat complex, chronic, and acute wounds, allowing for decreased risk of infection and healing. Quality wound care is possible at home.
