Medication Management

Get help managing medications. Our RNs and LPNs can help manage medications and schedules, set-up medication in boxes or dispensers, monitor and order refills, and ensure medications are taken in the proper dose and at the correct times each day. Getting medications right consistently is a critical part of any care plan, and it’s possible at home.

Monitoring Vital Signs and Oxygen

Making vital sign monitoring convenient. Our RNs and LPNs are available to manage vital signs such as blood pressure, temperature, oxygen saturation and pulse for both general health and also for specific health conditions like heart disease or hypertension. Consistent monitoring and, when necessary, reporting to a primary care physician is possible at home.

Catheter and Ostomy Cares

Specialized help is available for catheter and ostomy care. Our RNs and LPNs are able to perform regular maintenance and monitor changes to avoid complications. And, they are able to educate and train you or a family member to perform or assist with catheter and ostomy care so it can be managed at home.

Therapy for independence and safety. Our Occupational Therapists design a customized plan of care specific to the physical, emotional, and functional environment you or your loved one are in. The result is routines and strategies for managing daily activities safely and productively, which keeps people comfortably at home.

Falls Prevention Program

Get peace of mind by addressing the risk of falling. Our Falls Prevention Program is designed to provide screening, instructions, practical interventions, and resources, creating awareness and strategies for avoiding falls. Lymphedema Treatment

Our Occupational Therapist support lymphatic vessels and nodes by swelling reduction, skin protection and improved mobility. Using gentle lymph drainage, compression support, skin care education and personalized self- managed tools we help ease discomfort, reducing heaviness and lowering risk of infection or complications, all from the comfort of your home.
